By using appliedholistichealth.com (the "Site"), operated by Applied Holistic Health ("we," "us," or "our"), you agree to these terms. If you do not agree, please do not use the Site.
Educational information only, not medical advice
The Site, blog, guides, and other content are for general education. They are not medical advice, diagnosis, or treatment, and they do not replace care from your own healthcare provider. Never ignore professional advice or delay seeking it because of something you read here.
In an emergency
If you think you are having a medical emergency, call 911 or go to the nearest emergency room. Do not use the Site, email, or the contact form for urgent needs.
No patient relationship from the Site
Visiting the Site, reading content, sending a message, or booking a free discovery call does not create a provider-patient relationship. That relationship begins only after you complete the intake, consent, and scheduling steps for a visit or program with us and we agree to care for you.
Telehealth and where we can practice
Virtual visits are available only to people located in states where the clinician is licensed to practice when the visit takes place. In-person visits are at 702 Merrillville Rd, Crown Point, IN 46307. Separate consent forms apply to telehealth and to treatment.
Programs, memberships, and payments
Prices, services, and program details on the Site may change. Sign-up and payment for memberships and visits are handled through our partners (such as MyPrivate.md), and their terms also apply.
Cancellation and refunds
To cancel a membership or program, you must give us 30 days' notice. Monthly membership fees are non-refundable and are not pro-rated in the month of termination. Enrollment fees are non-refundable. Monthly fees continue to accrue until we receive your written cancellation. If you are a member, your signed Member Agreement also applies, and if it differs from this page, the Member Agreement controls.
Results are not guaranteed
Testimonials and examples describe individual experiences. Results vary, and we cannot promise any particular outcome.
Our content and your use of it
The text, images, logos, and materials on the Site belong to us or our licensors and are protected by law. You may view and share links to the Site for personal, non-commercial use. You may not copy, sell, or republish our content without written permission. Please do not misuse the Site, attempt to disrupt it, or submit unlawful or harmful content.
Links and third-party services
The Site links to services we do not control. We are not responsible for their content or practices.
Disclaimer of warranties
The Site is provided "as is" and "as available," without warranties of any kind, to the fullest extent the law allows.
Limit of liability
To the fullest extent the law allows, we are not liable for indirect, incidental, or consequential damages from your use of the Site or reliance on its content. This does not limit liability that cannot be limited by law, including for the care a patient receives from us under a signed treatment relationship.
Governing law
These terms are governed by the laws of the State of Indiana. Any dispute will be heard in the courts of Lake County, Indiana.
Changes
We may update these terms. Continued use of the Site after the effective date means you accept the changes.
